  • Abuse or Neglect:  A county child protective services staff has made a finding, related to the child care center, staff member or household member, of child abuse or neglect.  

    Administration and Records:  Most often used for violations related to the terms of the license, child and staff records, reporting and operational requirements.  

    Caregiver Background Check:  For violations relating to obtaining and documenting appropriate and complete background checks.

    Client Rights:  May be used for violations related to confidentiality

    Confinement/Physical Restraints:  For violations of the rule prohibiting physical restraint, binding, or tying to restrict movement or enclosing in a confined space such as a closet, locked room, box or similar cubicle.  

    Health/Medical:  For violations of rules related to child or staff health records, medical authorizations, policies regarding ill children, injuries, etc.

    Inappropriate Discipline or Behavior Management:  For violations of rules related to child guidance.  

    Licensed Capacity:  For violations related to the maximum number of children that may be cared for at any one time.  

    Nutrition:  For violations related to the type, amount, frequency, or documentation of meals and snacks.  

    Other:  Please contact the regional office for specifics.  The rule violation did not clearly fit another category.  

    Physical Plant:  For violations related to the building, space, temperature, hazards, fire protection, water, exits, furnishings, toilet facilities, outdoor play space, swimming areas.  

    Program:  For violations related to children's activities, such as lack of appropriate activities or equipment.   

    Safety Hazards:  For violations related to lack of protection from dangerous situations or objects.  

    Sanitation/Cleanliness:  For violations related to diapering, handwashing, clean environment and equipment, etc.  

    Staff Qualifications:  For violations related to educational and experience requirements for child care providers, including documentation.  

    Supervision and Staff Coverage:  For violations related to failure to provide adequate or appropriate supervision; or failure to have the number of staff necessary for the number of children in care.  

    Transportation:  For violations related to vehicles, drivers, safety seats/seatbelts, emergency information in vehicles.
