Paternal Interest Registry: Forms
Do not file forms electronically. A Declaration of Paternal Interest must be filed by mail or in person, because an original notarized signature of the declarant, and if the declarant is a minor, the declarant's parent or guardian, must be provided to the Registry. A Court Inquiry on Declaration of Paternal Interest must be filed by
