Rationale for Reasonable Suspicion of Program Violation:

The Department of Children and Families suspended this child care provider from the Wisconsin Shares program due to a conviction of the licensee for a crime substantially related to the operation of a business.

The provider had previously received citations from licensors related to the maintenance of attendance records on September 10, 2008, March 10, 2009, and June 16, 2009, including multiple no access visits and visits where children are signed in but not present.

Licensee has been convicted of the following:

  • 1-28-87: 943.20 - Theft - Charges disposed of under 943.24(2) Issue of Worthless Checks (=>$500) Convicted 3-25-91 and sentenced to pay restitution, 9 months in jail which was stayed and 2 years probation
  • 2-22-91: 943.24(2) - Felony - Issue of Worthless Checks (=>$500) - Charges disposed of under 943.24(1) Issue of Worthless Checks (<$500) Misdemeanor
  • 1-7-98: 49.95(1) - Felony - Public Assistance Violation - Disposition not shown on DOJ report. However the Judgment of Conviction (JOC) was obtained by our Dept and the document is on file. The JOC shows that Ms. Miller was found Guilty on 5-12-99 and sentenced on 8-26-99 to pay restitution in the amount of $15,990.00, 7 years in prison (stayed) and 5 years probation.
