Rationale for Reasonable Suspicion of Program Violation:

The Department of Children and Families suspended this child care provider from the Wisconsin Shares program due to a combination of red flags signifying Shares violations. These were identified through data and in person visits.

As of mid September, 2009, the provider had 39 Shares authorizations for a facility licenses to serve a total of 16 children. Of those 39 authorizations, 29 authorizations, or 74%, were for children of employees of Destiny's Dreamland Child Care Center.

During in-person visits by DCF staff on September 24, they found that at the first facility, it took a long time for the provider to answer the door. The provider was alone with zero children. At the second facility, five children were present, and provider could not provide a complete set of attendance records.

The provider has a licensing history of attendance related violations. The Department cited the provider for attendance violations on March 11, 2008 and March 31, 2008, in both cases because present children were not signed in. On September 24, 2008, the Department of Children and Families cited attendance violations because dates on the attendance record incorrectly documented two children; no departure time was recorded on September 23 for one child; the attendance record was pre-filled for one child on September 25; and the attendance record for the day of the visit showed one child in attendance from 8:30AM to 4:30PM, despite the fact that the child was not present. The Department also attempted to make licensing visits on February 26, 2008 and April 22, 2009 to the provider's second location, but was unable to gain access both times.
