Rationale for Reasonable Suspicion of Program Violation:
The Department of Children and Families and Racine County suspended this child care provider from the Wisconsin Shares program due to a combination of red flags signifying Shares violations. On 05/05/09, Racine County received a fax from customer stating that she ended her children's enrollment at the provider on 01/30/09. According to customer this was also the last day her children attended this child care. She received information that the provider was still recording attendance for her children after this date. Racine County conducted an investigation and determined the provider recorded attendance for the customer's children from 01/31/09 - 03/21/09. The sign-in/sign-out sheets show dates and times the children were in attendance but the sheets do not have signatures from the customer confirming these dates and times.
