• Participant arrives at Job Center to apply for W-2 Services
  • Goes to Receptionist to schedule an appointment and is given either the OQ™-10.2 or Rosenberg Scale for self-administration and a W-2 application.
  • Meets with Resource Specialist. The Resource Specialist determines needed services through interactive interview and scores the OQ™-10.2 or Rosenberg Scale.
  • The Resource Specialist will make the referral for a comprehensive clinical assessment if the score on the screening tool warrants further assessment. Only participants agreeing to participate in W-2 activities will be referred. If the participant wants to follow through with formal W-2 services, the Resource Specialist will indicate on CARES screen CMCC that the referral was made, and also indicates the referral on the paper W-2 application in the case comments section.
  • Participant has initial meeting with the FEP to complete the interactive interview and sign additional W-2 forms. The FEP informs the participant that the completion of the comprehensive clinical assessment must be part of their Employability Plan (EP).
  • The EP is developed and activities assigned for the participant that include the completion of the comprehensive clinical assessment and the recommendations from the comprehensive clinical assessment. The activity entered in CARES on the WPCS screen for AODA services is the component code CA. If a participant is concurrently involved in AODA and mental health activities the component status is CA and CM.
  • The OQ™-10.2 or Rosenberg is re-administered upon EP review or as needed. Modifications to EP are made if warranted.
